In order to receive a Gaming Activity Report statement regarding your tracked play at Ameristar Kansas City, please submit a completed, signed Gaming Activity Report Request Form.
A completed Gaming Activity Report Request Form may be submitted for processing in one of two ways:
- By mail to address to listed on form*
- In person at the Boyd Rewards Club
*Notary not required if form is requested or presented in person. Please allow 2 – 4 weeks for request processing.
